Care Pathway

As an independent specialist service we can provide an individual care pathway, responsive to need and ability to cope. We believe our purchasers, the individuals we support and their families want treatment, clearly defined pathways and a positive memorable experience.

We have the ability to have work at a pace suited to the individual, according to need and goals set out by them, with input from our Multi Disciplinary Team (MDT), key people in that person’s life and commissioners.

Our Multi Disciplinary Team (MDT) is based at Jeesal Cawston Park ensuring it is responsive and reactive with daily input and systems for review.

We provide a comfortable environment with expert clinical input, which will ensure future transition to a more independent phase of life, or move on from our service is a success.
